Oska Health, a hybrid care provider that combines personal health coaches with AI, has secured €11 million in seed funding to expand its continuous care model for high-risk patients with chronic conditions.

The round was led by Capricorn Partners and SwissHealth Ventures, with backing from Revent, Calm Storm, LBBW Venture Capital, BMH, GoHub Ventures and Aurum Impact.

Chronic diseases continue to strain healthcare systems globally. While clinical care is often well structured, many patients struggle to maintain healthy behaviours between doctor visits.

Studies show that around half of chronically ill patients do not take medications as prescribed and most fail to sustain long term lifestyle changes.

Oska Health addresses this gap by delivering personalised digital support between appointments. The company focuses on multimorbid patients those living with multiple chronic conditions such as chronic kidney disease diabetes and hypertension impacting millions across Europe.

Its approach blends human coaching with AI powered care to support patients at scale.

Through video calls, chat, and a dedicated digital therapy app, certified health coaches provide ongoing guidance on nutrition, exercise, and medication adherence.

The aim is to help patients turn medical advice into daily habits improve health literacy and reduce complications and preventable hospitalisations.

The fresh funding will help speed up the expansion of Oska’s AI-driven care infrastructure, designed to support and enhance care teams. The platform has already been assisting patients with chronic conditions for over three years.

CEO and co-founder Niklas Best emphasised that effective chronic care is driven by everyday habits rather than occasional medical consultations:
